For your convenience, send your prescription renewal request on-line for a speedy response. You may also place your request by calling our prescription line at (610) 374-2214 option 5.

Mammogram Request

For your convenience, send your mammogram request to one of our triage nurses. You may also place your request by calling our mammogram line at (610) 374-2214 option 4.

Please remember to bring with you to your appointment:

     Any information forms we have previously given to you
     All patients must present their valid photo ID to the receptionists at check in.

     Your insurance cards (we need to obtain the insurance subscribers name, S.S.# and D.O.B. to submit insurance claims)

     Copay

     Any outstanding balances

     Notify the staff if your address, phone numbers or insurance has changed

     Notify the staff if you need to update/change your HIPAA privacy information including who we can share your health information with

     Write down all current medications including dosages

     Write down questions/concerns you would like addressed at your visit

      Women's Clinic, Ltd. reserves the right to charge a patient a no-show fee to anyone canceling an appointment less than 24-hours.

» Dr. Minassian has been named to the publication committee of the Androgen Excess-PCOS Society, an exclusive international society of over 200 experts in the treatment and research of Polycystic Ovarian Syndrome. He will be performing quarterly reviews of the most vital research in the world medical literature in that disease. This important project will help society members worldwide to focus on the most current breakthroughs in the specialty, and he will be offering his expert opinion on each article.
